Each year the Delaware End-of-Life Coalition seeks Nominees for our Annual Awards (see categories below). Individuals nominated are those who have made outstanding contributions to improving the care for people at the end-of-life, or those grieving those who have passed.
Award Criteria
Nominations may be for any individual who goes above and beyond for those affected. These awards are intended to honor exemplary people – those that take more initiative, work harder, are more caring and thoughtful, and give more of themselves for families and those in need. Each professional nominee must be a current practioner in the field in which she/he is being nominated.
Award Determination
The Annual Awards Committee determines the recipients of these awards. Award recipients are determined SOLELY on the content of the written nominations. As such, the more information provided in the nomination submission, the higher the likilihood of your nominee being selected.
Please note the deadline for the submitting award nominations is 2/15/2013.
