ADDRESSING SPECIAL NEEDS OF VETERANS AT END-OF-LIFE
PURPOSE:
- To improve outreach to homeless and rural veterans in need of palliative and hospice care
- To educate and sensitize first responders, health care providers, and homeless shelter staff to the special needs of veterans, especially rural and homeless veterans at end-of-life
- To give program participants the tools to connect veterans who need end-of-life care
- To educate program participants on the referral process for hopice and palliative care and for the homeless program at the Veterans Administration
